METHOD 1:

  1. Download a Word text file here: AGREEMENT ICON
  2. The file will appear on your desktop
  3. Print it out, fill in by hand, include your cheque and send to Jackie Howard at

    Slaidburn Village Hall
    The Green
    Slaidburn
    Clitheroe
    Lancashire BB7 3ES

METHOD 2:

  1. Download the PDF file here: PDF LOGO
  2. Proceed as in method 1 or complete on screen by typing in spaces or clicking boxes. (Note you cannot save the completed form on your computer and will need to run off as many hardcopies as you need)
  3. Print out, sign and post with your deposit cheque to Jackie Howard at:
